Heart failure remains the leading cause of hospitalization for person's ? 65 years of age, and rates of hospital readmission within 6 months are as high as 50% with an average hospital stay of approximately 6 days (AHA, 2010). In 2010, it was estimated that the cost of care for HF in the United States was approximately $39.2 billion, which represents 1-2% of all healthcare expenditures. With the alarming re-hospitalization rates and increased cost of care among heart failure patients, there is a compelling need for more effective post-acute care management.
Nurse practitioners (NPs) are competent to make significant contributions to the clinical management of HF patients by providing care that is comprehensive, collaborative, and cost effective. Studies have shown that NP-directed, multidisciplinary, HF management programs have a positive effect on patient outcomes, and the programs decrease readmission rates and length of hospital stay. However, there is lack of research studies on NPs' knowledge and utilization of published HF guidelines as well as published standards on care processes and structural elements of an NP-led HF clinic.
On August 19, 2011, a postcard with a link to the Kibot Questionnaire was mailed to 2, 000 nurse practitioners licensed in 1 of the 50 states. Nurse practitioners were equally recruited at the Advance Practice Providers second annual meeting held in Fargo, ND, on September 17, 2011. Findings from the study suggested that only 61% (n = 49) of the NPs surveyed were familiar with the ACC/AHA guidelines for diagnosing and managing patients with HF. Only 11% (n = 46) agreed to following the guidelines "All the time." Nonetheless, 68% (n = 46) of the NP respondents utilized a HF specialty clinic.
Based on the review of literature and the findings from the survey, the essential domains of the nurse practitioner managed HF clinic were proposed. The components of the Chronic Care Model were used as a guiding framework for designing the HF clinic. Utilizing a disease-management HF clinic will ensure comprehensive education and counseling that is individualized to patients' needs, optimize medical therapy, and implement a mechanism to ensure appropriate follow-up
